Please read the Child Protection Policy Fact Sheet and print, complete, sign and return the Child Protection Policy Screening Forms.

Reference Form

Please print three copies of the Reference Form and then give one form to your pastor or church leader, one form to another mature Christian adult (CEF worker, Christian Service Coordinator, Church Member, etc.) and one form to a friend.

When distributing reference forms to the above list, please include a self-addressed (to CEF), stamped envelope to facilitate a quick and simple return process.
